INSURANCE & PAYMENT
My full out-of-pocket fee for a 50 minute session is $125
Payment is due at time of service (including insurance co-pay, fee before your deductible is met, etc.) unless previous arrangements are made. I accept: Cash, & all Major Debit & Credit Cards
I am credentialed to accept Anthem Blue Cross Blue Shield, Cigna, United (UBH/Optum & UMR), Beacon Health Options/Kaiser, New Directions EAP
Questions you'll want to ask your insurance:
--Ask if you have Mental/Behavioral Health Benefits on your policy for outpatient/seeing a psychotherapist/LPC in their office. If they ask for a procedure or CPT code you can let them know the potential codes will be 90791, 90837 or 90834.
If you have insurance, but I don't accept your current provider, it could be helpful to reach out to your provider to see if you have "out of network" options for reimbursement. I am happy to provide a superbill for you to send to your insurance & my full fee will be due at time of service.
